For Neck Pain, Chiropractic and Exercise Are Better Than Drugs

Seeing a chiropractor and engaging in light exercise relieves neck pain more effectively than relying on pain medication, new research shows.

 

The new study is one of the few head-to-head comparisons of various treatments for neck pain, a problem that affects three quarters of Americans at some point in their lives but has no proven, first-line treatment. While many people seek out spinal manipulation by chiropractors, the evidence supporting its usefulness has been limited at best.

Rx drug deaths triple in decade

 

Inappropriately used prescription pain medications kill 15,000 people in the United States each year, according to a new report released by the Centers for Disease Control and Prevention. "We're in the midst of an epidemic," says CDC Director Dr. Thomas Frieden. "Narcotics prescribed by physicians kill 40 people per day."

Prescription drug deaths now outnumber traffic fatalities in U.S., data show

 

Propelled by an increase in prescription narcotic overdoses, prescription drug deaths now outnumber traffic fatalities in the United States, a Times analysis of government data has found.

 

Prescription drugs exceeded motor vehicle accidents as a cause of death in 2009, killing at least 37,485 people nationwide, according to preliminary data from the U.S. Centers for Disease Control and Prevention.

 

While most major causes of preventable death are declining, prescription drugs are an exception.  The death toll has doubled in the last decade, now claiming a life every 14 minutes.  By contrast, traffic accidents have been dropping for decades because of huge investments in auto safety.

 

Public health experts have used the comparison to draw attention to the nation's growing prescription drug problem, which they characterize as an epidemic.  This is the first time that prescription drugs have accounted for more fatalities than traffic accidents since the government started tracking drug-induced deaths in 1979.

Hidden under Tom Engel's skin is a small device that around the clock pumps a morphine-like drug into his spinal column and ultimately into his brain.

 

Without it, his back pain would be unbearable.

 

Before getting the pump, Engel underwent two surgeries in which a product known as Infuse -- bone morphogenetic protein-2 or BMP-2 -- was implanted in his spine in an effort to relieve his back pain.

 

The first surgery, in 2002, used the product according to the label indications approved by the Food and Drug Administration. The second, in 2004, used the product in an unapproved "off-label" way.

 

A Pain that Knows No End

 

"He doesn't even go to church anymore," said his wife, Rita. "We went to church all our lives. He can't walk that far and the drugs put him to sleep."

American Chiropractic Association Suggests Conservative Care Before Prescription Drug Use

 

In a new survey, chiropractic care outperformed all other back-pain treatments including prescription and over-the-counter medications, deep-tissue massage, yoga and Pilates. According to the American Chiropractic Association (ACA), this news reinforces the use of conservative care options as a first line of defense against pain.

 

The survey, released by a leading consumer product rating and survey publisher in late July, found that 65 percent of those surveyed using chiropractic care for back pain said it helped a lot. Only 53 percent of respondents using prescription medications found them as helpful.

Army on brink of new ways to fight pain

SAN ANTONIO — The Army is reinventing the way it treats pain.

 

Based on 109 recommendations from an Army-led Pain Management Task Force, the Army is seeking to move away from merely dispensing pain medication to embracing a holistic, multidisciplinary approach to caring for soldiers.

 

This approach could include traditional medications coupled with more unconventional treatments such as massage therapy, acupuncture and yoga.

Overall, studies have shown that spinal manipulation can provide mild-to-moderate relief from low-back pain and appears to be as effective as conventional medical treatments. In the 2007 guidelines  [494 KB PDF],  the American College of Physicians and the American Pain Society include spinal manipulation as one of several treatment options for practitioners to consider using when back pain does not improve with self-care.

Government Report Notes Benefits of Spinal Manipulation

The National Prevention Council (NPC), recently called for the development of a National Prevention Strategy  to outline goals and recommendations for improving health and wellness for all Americans. The National Prevention Strategy highlights several preventative measures throughout the report including alternative therapies for back and neck pain. 

  

According to the report, one key aspect of expanding preventive services is to “enhance coordination and integration of clinical, behavioral and complementary health strategies.” The report also specifically touts the benefits of spinal manipulation stating, “Complementary and alternative therapies for back and neck pain (e.g., acupuncture, massage, and spinal manipulation) can reduce pain and disability.” Read the full report here.

The Detroit edition of Examiner.com recently highlighted research published in the April 2011 issue of the Journal of Occupational and Environmental Medicine. The study, “Health Maintenance Care in Work-Related Low Back Pain and Its Association with Disability Recurrence," concluded that chiropractic care was more effective  for common work-related low-back pain in comparison to treatments by physicians or physical therapists. In total, chiropractic patients demonstrated lower medical expenses, fewer disability recurrences and shorter initial periods of disabilities.

Effectiveness of Spinal Manipulation for Chronic Low Back Pain

Acute or sub-acute low back pain patients have been the target of the preponderance of past randomized clinical trials of spinal manipulation.  Fewer studies have used chronic pain patients.  A study published on May 27, 2011 from Northwestern Health Science University randomly assigned 301 chronic low back pain patients to one of three treatment arms consisting of spinal manipulation, home exercise, or supervised exercise.  The study found that there was no significant difference in the three groups with respect to pain and several other outcomes.  Patients that received the supervised trunk exercises “were most satisfied with care and experienced the greatest gains in trunk muscle endurance and strength”.

Medical Research Supports Chiropractic Maintenance Care

Two research studies generated by the medical profession this year add evidence in support of the value of chiropractic maintenance care.

 

The first study published in January 2011 in Spine concluded that “SMT is effective for the treatment of chronic non specific LBP. To obtain long-term benefit, this study suggests maintenance spinal manipulations after the initial intensive manipulative therapy.”

 

The second study in April 2011, published in the Journal of Occupational and Environmental Medicine, provides additional support for the value of chiropractic maintenance care for post injury low back pain patients.  The study followed 894 injured workers for a period of one year.  During that year there were four different types of therapy available to the workers:  medical management, physical therapy, chiropractic, and no therapy.  Episodes of repeat disability were recorded during the year following the initial injury.  Physical therapy had the highest percentage of reinjured workers followed by those receiving medical management or no treatment at all.  The lowest incidence of repeat injury was found among those workers who had received chiropractic maintenance care.

Spinal Manipulation Proves Equally Beneficial As Surgery In Sciatica Treatment

In a recent study, "Manipulation or Microdisketomy for Sciatica? A Prospective Randomized Clinical Study," (Journal of Manipulative and Physiological Therapeutics, October 2010, Vol. 33 Iss. 8, p: 576-584), researchers concluded that spinal manipulation was just as effective as microdiskectomy for patients struggling with sciatica secondary to lumbar disk herniation (LDH). The patient population studied included people experiencing chronic sciatica (symptoms greater than six months) that had failed traditional, medical management. Overall, 60 percent of patients who received spinal manipulation benefited to the same degree as those who underwent surgery.

RAND Awarded Grant to Evaluate Chiropractic Care in Treating Military Members

The RAND Corporation, the Palmer College of Chiropractic, and the Samueli Institute have been awarded a $7.4 million grant from the Congressionally Directed Medical Research Program to conduct clinical trials of the effectiveness of chiropractic care in treating a variety of health problems among military personnel.

 

The Congressionally Directed Medical Research Program administers the Congressional Special Interest Medical Research Programs encompassing breast, prostate, and ovarian cancers, neurofibromatosis, military health, and areas specified by Congress. Research grants are awarded through a two-tier peer and programmatic review process.

 

The project will be conducted over a four-year period and will be directed by Ian Coulter, the Samueli Institute Chair in Policy for Integrative Medicine at RAND.

 

The project will include clinical trials that examine the effect of chiropractic clinical treatment, either exclusively or as an adjunct to other treatments, on health concerns relevant to military members, including non-surgical orthopedic injury, pain management, smoking cessation, injury prevention and enhancing fitness for duty. The project will involve military sites throughout the country.

 

The project will be conducted through the RAND Center for Military Health Policy Research.

House Panel OKs Primary Care From State Chiropractors

ACA Member Dr. Christine Goertz to Oversee the Largest Clinical Trial of Chiropractic to Date

 

The American Chiropractic Association (ACA) today announced that ACA member and research consultant Christine Goertz, DC, PhD, will oversee the design and implementation of the three clinical trials funded by a $7.4 million grant awarded to scientists at the Palmer Center for Chiropractic Research (PCCR), the RAND Corporation and the Samueli Institute by the Congressionally Directed Medical Research Program.

 

The grant will fund a four-year research project to assess chiropractic treatment for military readiness in active-duty personnel. This is the largest single award for a chiropractic research project in the history of the profession and will be used to conduct the largest clinical trial evaluating chiropractic care to date.

Study Finds That for Low Back Pain, Starting with Chiropractic Saves 40% on Care

A new study finds that care for low back pain initiated with a doctor of chiropractic (DC) saves 40 percent on health care costs when compared with care initiated through a medical doctor (MD), the American Chiropractic Association (ACA) announced today. The study, featuring data from 85,000 Blue Cross Blue Shield beneficiaries, concludes that insurance companies that restrict access to chiropractic care for low back pain treatment may inadvertently pay more for care than they would if they removed such restrictions.

 

Low back pain is a significant public health problem. Up to 85 percent of Americans have back pain at some point in their lives. In addition to its negative effects on employee productivity, back pain treatment accounts for about $50 billion annually in health care costs—making it one of the top 10 most costly conditions treated in the United States.

 

Published in the Journal of Manipulative and Physiological Therapeutics (JMPT), the new study, “Cost of Care for Common Back Pain Conditions Initiated With Chiropractic Doctor vs. Medical Doctor/Doctor of Osteopathy as First Physician: Experience of One Tennessee-Based General Health Insurer,” looked at Blue Cross Blue Shield of Tennessee’s intermediate and large group fully insured population over a two-year span. The insured study population had open access to MDs and DCs through self-referral, and there were no limits applied to the number of MD/DC visits allowed and no differences in co-pays.

 

Results show that paid costs for episodes of care initiated by a DC were almost 40 percent less than care initiated through an MD. After risk-adjusting each patient’s costs, researchers still found significant savings in the chiropractic group. They estimated that allowing DC-initiated episodes of care would have led to an annual cost savings of $2.3 million for BCBS of Tennessee.

 

“As doctors of chiropractic, we know firsthand that our care often helps patients avoid or reduce more costly interventions such as drugs and surgery. This study supports what we see in our practices every day,” said ACA President Rick McMichael, DC. “It also demonstrates the value of chiropractic care at a critical time, when our nation is attempting to reform its health care system and contain runaway costs.”

 

The full study is available online and will appear in print in the December issue of Journal of Manipulative and Physiological Therapeutics.

Back spasms subsiding for Harang

CINCINNATI -- Wednesday was a "really good day" for Aaron Harang, who is finally starting to get some relief from lower back spasms that have sidelined him all month, and he may begin throwing again in the next few days.

 

Harang -- who last pitched June 30 -- said he went to the chiropractor Wednesday and got an adjustment that paid off.

Common Pain Relievers Raises Heart Risk In Otherwise Healthy Patients

On May 24th, the U.S. House of Representatives passed the Chiropractic Care Available to All Veterans Act.  The next day, Army Times covered the story.  Rep. Bob Filner, chairman of the Houlse Veterans' Affairs Committee and chief sponsor of the bill, is a strong chiropractic supporter.  Musculoskeletal disorders "are the single most common ailment facing returning veterans, " Rep. A new Macquarie University study involving two Australian Rules football clubs has found that chiropractic treatment can significantly reduce the risk of players succumbing to hamstring injuries and lower limb muscle strain.

 

Two semi-elite Victorian Football League (VFL) clubs participated in the research, which is the first Australian study to examine the role of chiropractic treatment in minimising injury in footballers.

Washington (ANI): Pop Queen Madonna has added a top chiropractor to her massive touring entourage, it has emerged. Dr. Steven Margolin has joined the 50-year-old singer on her 'Sticky and Sweet' world tour, according to reports.

 

"It's (tour) going to be very draining, and she needs him around to aid her in her recovery time after each show," Contactmusic quoted a source, as telling America's LifeStyle magazine. "She'll have daily massages

to relax her,' the source added.

      A recent report on the June 15 CBS Early Show recommended chiropractic as a potential treatment option for children with attention deficit hyperactivity disorder (ADHD).

      In speaking with CBS anchor Julie Chen, medical correspondent Dr. Jennifer Ashton noted that some studies have shown alternative treatments such as chiropractic and dietary changes to be beneficial for children with ADHD.

     The report stemmed from a recently released study in The American Journal of Psychiatry suggesting that children and teens who take stimulants like Ritalin for ADHD have an increased risk for sudden cardiac death; however, the FDA says the study has major limitations.

Chronic back pain is among the most common patient complaints. Its prevalence and impact have spawned a rapidly expanding range of tests and treatments. Some of these have become widely used for indications that are not well validated, leading to uncertainty about efficacy and safety, increasing complication rates, and marketing abuses. Recent studies document a 629% increase in Medicare expenditures for epidural steroid injections; a 423% increase in expenditures for opioids for back pain; a 307% increase in the number of lumbar magnetic resonance images among Medicare beneficiaries; and a 220% increase in spinal fusion surgery rates. The limited studies available suggest that these increases have not been accompanied by population-level improvements in patient outcomes or disability rates. We suggest a need for a better understanding of the basic science of pain mechanisms, more rigorous and independent trials of many treatments, a stronger regulatory stance toward approval and post-marketing surveillance of new drugs and devices for chronic pain, and a chronic disease model for managing chronic back pain.  full story here.....

Routine Lumbar Imaging No Help for Low Back Pain

 

PORTLAND, Ore., Feb. 5 -- Routine imaging to assess acute or subacute low back pain holds no clinically relevant benefits for patient outcomes, researchers here affirmed.  

 
Patients with low back pain who had radiography, MRI, or CT scans done without a clear indication had no significant improvement in pain or function in the short or long term through 12 months compared with patients who did not have imaging done, Roger Chou, M.D., of the Oregon Health and Science University here, and colleagues reported in the Feb. 7 issue of The Lancet.
